If you have ANY other insurance carrier (example - CIGNA, UNITED, AETNA) - we are OUT OF NETWORK and your visit will be at Self-pay rates ranging from $250 to $350 for a new patient visit and follow up visits are $175 to $250 depending on the appointment type. “WE REQUIRE A $60 BOOKING FEE FOR ALL NEW PATIENTS, OF WHICH WILL BE APPLIED TO THE OVERALL COST OF YOUR VISIT” It is non refundable in the case of no-show or late cancellation within 24 hours of your appointment time. Attention:  Dr. Reyes is only accepting telemedicine patients through the virtual telemedicine platform, Dr. Alex Reyes LLC.  You will be required to pay at time of booking and Dr. Alex Reyes LLC does NOT bill any insurance carriers.  You are welcome to use your insurance for any lab testing, or other doctor’s orders, just like any doctor’s office.
